HIV/AIDS guidelines

I am confused on the HIV AIDS guideline if the doctor documents history of hiv on antivirals it codes to b20 And long term use . What about a pt with a dx of HIV ILLNESS on antivirals? Will this be Z21 or B20 please?
is the antivirals what changes this code ? Thank you
