Is there a guideline regarding a patient active disease and a family history Of the same disease. Example why would you or wouldn‘t you code a patient with an active htn I10 and a family hx of htn (circulatory) Z82.49 Together ?

physicians should report all present, relevant diagnosis. ICD guidelines instruct to choose a primary diagnosis (to describe the main reason for the visit/encounter), and to “List additional codes that describe any coexisting conditions.”
What this means is, physicians should code for all documented conditions that co-exist at the time of the encounter/visit, and require or affect patient
care treatment or management.
