I've finished my course at my college and I'm going back through Buck's step by step in preparation for CPC exam, and it asks for the ICD-10-cm code for the following information from a sample operative report:

Pre-op DX: Left ischial pressure ulcer w massive ischioperineal & buttock sinus

Post-op: same

Findings: there was a 2 cm open surgical ulcer extending down and connecting with an 8 x 30 cm diameter granulation-lined sinus cavity.

Surgical procedure: Excision of left ischial ulcer with total excision of 8 x 30 cm sinus of the buttock, perennial, and ischial areas.

The answer bank is showing it is L89.224 (Ulcer, pressure, hip)

I was wondering why the answer is coded to the hip and not the buttock (L89.324) or under "contiguous site of back, buttock, hip" (L89.44) ?20230823_095435.jpg

The answer key could be incorrect as I see L98.419 as an ulcer of the buttock.
