Smoking Cessations w/ F17.200 (Nicotine dependence) – I am wondering whether it is acceptable to report a behavioral health category F17.xxx based on a questionnaire filled out by a medical assistant in an effort of trying to support a patient with smoking cessation. Although there may be a financial incentive to provide this counseling, I have noticed that the physician's note often lacks documentation. When I have asked about it, I have been informed that the MA has the necessary qualifications. I would greatly appreciate any feedback, as I have been unable to locate specific guidelines for this matter. Your insights would be invaluable and could potentially help me make significant progress.
I do not believe that the MA would be able to document or use documentation from a form. The provider still has to document the dx. I did find the link above which on page 12 provides documentation tips.
