Provider is billing a wellness visit without abnormal findings, additional EM of 99213 with 25 modifier Cpt 17110, 11200

I Believe the CPT 11200 Is incorrect. Is there any Derm coders there can help me with this?
 
Is the problem with 11200 (the second procedure outlined in the above note) in the wording of the provider? The procedure noted was skin tags however the procedure summary indicates "Destruction of 2 lesions". Is that the confusion? Do you think there is a better, more descriptive procedure code to use instead?
This may require a query to the provider. If the removal was of two skin tags - one on the breast and one of the glute - then 11200 would be correct. However, if they were lesions but used cryotherapy to remove them, then they are inclusive of the 17000 code already being submitted for the 1st procedure.
