Hello! I am currently working through AHIMA's online coding program (using the 2021 CPT manual) and am a little confused by a question in my textbook. Maybe someone could shed some light on it.

The question is: A physician excised an area 3.0cm x 2.0cm x 1.5cm from the patient's back. The pathology report shows a lipoma of the subcutaneous layer of skin.

According to the book, the correct code is 11403 (excised diameter 2.1 to 3.0cm).

Unless I am completely misunderstanding how to calculate the excised diameter, I think the code 11406 (excised diameter over 4.0 cm) is correct. Am I missing something or is my book wrong?
