CPT Assignment Paratubal Cystectomy.

Does anyone have suggestions as to a CPT code when a paratubal cystectomy is completed in conjunction with a cesarean? CPT assist guides to 58720 however no portion of the tube was removed; only the cyst

"A 4 cm simple paratubal cyst was noted adjacent to the left ovary. The cyst was drained then the cyst wall clamped cut and the mesosalpinx secured with vicryl tie."

we are finding so much conflicting info on the internet.

Answer:

I am inclined to agree with 58805. This seems to be the closest we can get. What makes what we do so exciting to have an adventure like this one. Maybe we can do this for a club recording or case study.

58805 - Drainage of ovarian cyst(s); unilateral or bilateral (separate procedure); abdominal approach



An ovarian cyst is a sac or pouch arising from the ovary. The cyst may be fluid-filled or contain semi-solid or solid material. Using a transvaginal approach; a vaginal ultrasound probe is used to identify internal structures and aid in localization of the cyst. A small incision may be made in the vagina. Alternatively; the needle may be advanced directly through the posterior vaginal fornix and into the ovarian cyst. The cyst is then aspirated and drained until all of the cyst contents have been removed. The transvaginal approach is typically reserved for medically fragile patients who are not good candidates for an open abdominal procedure. Using an abdominal approach; a suprapubic incision is made through the skin and subcutaneous tissue. Subcutaneous fat is cleared and the anterior rectus fascia incised. The rectus muscles are retracted and the underlying transversalis fascia and peritoneum exposed and incised. The ovary and cyst are exposed and inspected along with the uterus;
fallopian tubes and the contralateral ovary. The cyst is punctured and drained taking care to prevent spillage of cyst contents. The cyst contents are sent for separately reportable laboratory analysis.
