If she is Medicare and being seen in an outpatient facility then yes you can use G0463 - Hospital outpatient clinic visit for assessment and management of a patient.
Referral does not mean consult - to be a consult you need to have a request for an opinion; a rendering of an opinion and report back of the opinion.
