Hello,

Does anyone have the Anthem OHIO ABA policy? Trying to find out if 97155 is covered if a BCaBA is the QHP.

Does anthem cover ABA therapy?
Only ABA services for which authorization has been given will be covered. ... When requesting pre-authorization for outpatient ABA services, complete the Adaptive Behavioral Treatment Request Form. Note: As of January 1, 2019 the ABA assessment and treatment plan request form were combined into one form.
