In our practice we have a dietician that does group diabetic education for 1 1/2 hours and immediately after the session a nurse practitioner sees the patients individually for about 5 minutes to review medications and maybe a brief exam. She documents that the patient attended 1 1/2 hours of education and her review of medications and makes recommendations for medication changes and documents diagnosis or multiple diagnoses and then codes it as a 99214. Is a 99214 correct? I really don’t know how to code for these services. The dietitian does not bill individually, he bills under the nurse practitioner
 
If you are using MDM this can be done based on number of diagnoses and changes to POC. I wouldn't base on time for the NP and the dietician is doing different work.

It sounds like the dietician is being paid by the NP?
