So I would assume it would be based off off the 2021 guidelines, since that is the manual you will be using for your exam.

Yes, the exam is based on the guidelines of the year of the exam as well.

AAPC updates the exams each January.

 
Hi everybody I was supposed to take my exam a few weeks ago however, my father had a heart attack so I cancelled. I am now taking my exam in December my question is the books I have are 2020 I have made notes for some changes in them do you think this would be alright to use or do you suggest getting the newer books? I have my books already tabbed and notes in I just hate spending the money if there are only a few changes Thank-You

There are hundreds of code changes each year so it is always recommended you have the current year's manuals.

There were also new E/M guidelines this year.
