Therese S. asked:

For Rapid desensitization code 95180 billed each hour. Does the time up coding apply? If it is 91 minutes then bill as 2 units (2 hours). Or is it directly as description states each hour (60 minutes = 1 unit) and (120 minutes = 2 hours)? As necessary the start and final end time would need be documented accordingly to bill. Thank you!

Answer:

* My research indicates you would need a full hour. The idea with this desensitization a.k.a. cluster or rush immunotherapy is to inject antigens in increasing amounts at 30 to 120 minute intervals. You would bill 1 unit for each hour.

So 91 minutes would be just one unit of 95180 in my opinion. As soon as you reached 120 minutes or two hours then you could code two units.

CPT is funny with time codes - sometimes they let you code a part thereof and other times not. So in the ascense of the guidelines telling us we can I would go with the full hour understanding.
