Resolved Right ovarian cystectomy

I need some help with coding a laparoscopic right ovarian cystectomy this was done during a low anterior sigmoid resection the CPT code 58662 description is not matching the op report description listed this is what the physician is saying he did.

There was a chocolate appearing right ovarian cyst. It did have the appearance of endometriosis however could have been a hemorrhagic cyst as well. I then performed right ovarian cystectomy. There was a dark cyst adjacent to the right ovary. Using the robot vascular sealing device the cyst was separated from the surrounding tissue. The ovary seemed like it was mostly spared. There was some tube that seemed to be incorporated. We were very careful to confirm this was away from the ureter. Once the cyst was excised it was placed in a specimen bag with the string remaining out through the trocar.

The code 58662 seems to be for a laparoscopic ovarian cyst removal but there are also codes for a cystectomy as well perhaps that is the issue.

So is this a cyst removal only or a bladder removal too?

CPT 58662 is the correct code for Laparoscopic Ovarian Cystectomy.

58925 and 58611 are not laparoscopic procedures.

58611 should not be billed with modifier 59 because this is an add-on code and is performed in junction with a Cesarian section or other intra-abdominal surgery

Code 58662 (laparoscopy, surgical; with fulguration or excision of lesions of the ovary, pelvic viscera, or peritoneal surface by any method) would cover the removal of the left ovarian excrescences, but does not capture the lysis of adhesions. Many payers bundle this procedure because they believe it is incidental.
