The physician performs a radical excision of an external auditory canal lesion. Taking care to protect the facial nerve; an incision is made in the external auditory canal extending into the front of the ear to the parotid gland. The anterior portion of the external auditory canal lesion is excised along with portions of the parotid gland. Intraoperative frozen sections of surgical margins are sent for pathological examination. If margins are positive for tumor invasion; piecemeal resection of gross tumor extension is performed until all margins are free of all visible and microscopic evidence of tumor. The physician may remove skin; soft tissue; cartilage and bone. Separately reportable reconstruction of the external auditory canal with skin grafts is performed as needed. The submandibular gland on the affected side is removed. The anterior belly of the digastric muscle as well as the sternohyoid and sternothyroid muscles may also be removed.
