Hello,

Can you bill CPT 99221-99239 with POS 51? Or, these codes should be reported only with POS 21 and POS 22? Please advise. Thanks!

Based on my understanding, CPT codes 99221-99239 are used to report various levels of evaluation and management services provided in the hospital inpatient or observation setting. The Place of Service (POS) code indicates where the service was provided. POS 21 is for inpatient hospital services and POS 22 is for outpatient hospital services.

So in general:

- CPT 99221-99239 should be reported with POS 21 for inpatient hospital services.

- CPT 99221-99239 should be reported with POS 22 for outpatient hospital observation or emergency department services.

- POS 51 is for inpatient psychiatric facilities. So it would not be appropriate to report 99221-99239 with POS 51, as these E/M codes are for medical, not psychiatric, services.

In summary, CPT codes 99221-99239 should only be reported with POS 21 or 22 based on whether the setting was inpatient or outpatient hospital. POS 51 would not apply for these medical E/M services. Let me know if you need any clarification or have additional questions!

Good question. Here are some common CPT codes that could potentially be reported with Place of Service (POS) 51 for inpatient psychiatric facility services:

  • 90791 - Psychiatric diagnostic evaluation
  • 90792 - Psychiatric diagnostic evaluation with medical services
  • 90832 - Psychotherapy, 30 minutes
  • 90833 - Psychotherapy, 30 minutes with E/M service
  • 90834 - Psychotherapy, 45 minutes
  • 90836 - Psychotherapy, 45 minutes with E/M service
  • 90837 - Psychotherapy, 60 minutes
  • 90838 - Psychotherapy, 60 minutes with E/M service
  • 90839 - Psychotherapy for crisis, first 60 minutes
  • 90840 - Psychotherapy for crisis, each additional 30 minutes
  • 90846 - Family psychotherapy without patient present
  • 90847 - Family psychotherapy with patient present

The key is that these are psychiatric CPT codes, not general medical E/M codes.

Some other important notes:
  • POS 51 is only for inpatient psychiatric facilities.
  • If the patient is in a medical hospital but seeing psychiatry, POS 21 or 22 would apply.
  • Any E/M services provided in a psychiatric facility should be reported with the appropriate psychiatric CPT codes.

Let me know if this helps explain which codes are appropriate to report with POS 51!

Thank you for explanation! Under Psychiatry section of CPT book 2024 pg. 756 it states: ""Evaluation and management services (99202-99285, 99304-99337, 99341-99350) may be reported for treatment of psychiatric conditions, rather than using psychiatry services codes, when appropriate. Hospital care in treating a psychiatric inpatient or partial hospitalization may be initial or subsequent in nature (see 99221-99233)" This made me think that POS 51 was allowed with listed codes.
