Past Medical Hx + CAD

This might be a dumb question, but I sometimes struggle with knowing when a condition listed within the documentation of Past Medical HX should be coded as a current condition vs a 'personal hx of' code. I usually look to the list of patient's current medication to help me but sometimes it is not available.

Would love to get some feedback from others to help me clarify. For example, in this scenario, would the CAD be a dx code from Chp 9 or Chp 21?

"patient admitted to the hosp bc of hypoglycemic shock. She was confused about her insulin dosages and accidentally took twice the prescribed amount. Her past medical history includes coronary artery disease with stent placement last year."
