Resolved MolDx Z Code

Can anyone tell me where to enter the molDx z code on a UB? We bill as hospital outpatient and haven’t had any trouble until recently. Thank you

That is great but I think since MolDx zcodes only have to be added for certain labs, it is not listed on that fact sheet. I am really at a loss!

Effective March 1, 2017, hospitals must add the DEX Z-Code™ identifier in block 80 of the UB04 claim form or on line SV202-7 of the 837I electronic claim.

Thanks for looking. That is what we have been doing but recently the claims started rejecting and when we called we are told it is wrong but they are not permitted to tell us where to put it.

If nothing has clearly changed in the process, then maybe see if there is an I.T. problem causing the rejections?
