It is something that you learn with studying and experience; so there is no simple answer as to "how do you know when a modifier is needed; and which one."
A good place to start is Appendix A of the CPT manual with describes the various modifiers. Skim through that appendix and you will get an overview; but eventually you will want to read through it carefully. That will help you see the various uses of modifiers.
