Hello i need your help Can you please tell me more regarding MODIFIERS LC, LD AND RC. There is a note in my book chapter 8 cardiovascular system book 2021 " Do not use HCPCS Level II coronary artery modifiers LC, LD and RC to identify the bypass graft placement. These modifiers are used to indicate the specific vessel involved in percutaneous coronary artery procedures 92920-92444. My question is from what range of cpt codes can not these modifiers attached. I m very confused. Thanks
