An example was just shown on this slide but if you look on another slide on the E/M presentation or in the CPT manual the level P or E under Brief 1-3 really would be based on the note provided. It is a chronological description of the development of the patients present illness from the first sign and/or symptom to the present. This includes description of location; quality; severity;timing;context; modifying factors; and associated signs and symptoms.
