Chronic diseases
are defined broadly as conditions that last 1 year or more and require ongoing medical attention or limit activities of daily living or both. Chronic diseases such as heart disease, cancer, and diabetes are the leading causes of death and disability in the United States.

Sequencing guideline for acute and chronic conditions, if the condition has separate subentries with the same indentation level for acute and chronic in the Index of ICD-10, code both the acute and chronic condition, with the acute listed first followed by the chronic condition.

Co-morbidity​

Prevalence and Medicare spending are presented for chronic condition co-morbidity at the national level. Co-morbidity is illustrated by the combinations of the 21 chronic conditions. Dyads represent the combinations of two chronic conditions among Medicare beneficiaries with at least two of the conditions. There are 210 dyads. Triads represent the combinations of three chronic conditions among Medicare beneficiaries with at least three of the conditions. There are 1,330 triads.
