HI everyone, my practice is currently thinking of using Botox as a treatment option for migraines. Does anyone know what would be considered medically necessary for reimbursement purposes for Botox to be considered an appropriate treatment plan? As in how many times does a patient have to be diagnosed with migraines in order for it to be covered and what CPT/ DX codes should be used for the claim. Thank you.
